Background and objectives: Physicians are exposed to matters of existential character at work, but little is known about the personal impact of such issues. Methods: To explore how physicians experience and cope with existential aspects of their clinical work and how such experiences affect their professional identities, a qualitative study using individual semistructured interviews has analysed accounts of their experiences related to coping with such challenges. Analysis was by systematic text condensation. The purposeful sample comprised 10 physicians (including three women), (...) aged 33–66 years, residents or specialists in cardiology or cardiothoracic surgery, working in a university hospital with 24-hour emergency service and one general practitioner. Results: Participants described a process by which they were able to develop a capacity for coping with the existential challenges at work. After episodes perceived as shocking or horrible earlier in their career, they at present said that they could deal with death and mostly keep it at a distance. Vulnerability was closely linked to professional responsibility and identity, perceived as a burden to be handled. These demands were balanced by an experience of meaning related to their job, connected to making a difference in their patients’ lives. Belonging to a community of their fellows was a presupposition for coping with the loneliness and powerlessness related to their vulnerable professional position. Conclusions: Physicians’ vulnerability facing life and death has been underestimated. Belonging to caring communities may assist growth and coping on exposure to existential aspects of clinical work and developing a professional identity. (shrink)

Desmosomes are junctions involved in intercellular adhesion of epithelial cells and hemidesmosomes are junctions involved in adhesion of epithelia to basement membranes. Both are characterised at the ultrastructural level by dense cytoplasmic plaques which are linked to the intermediate filament cytoskeleton of the cells. The plaques strongly resemble each other suggesting a relationship between the two kinds of junctions, as implied by their names. Recent characterisation of the molecular components of the junctions shows they are, in fact, quite unrelated implying (...) that structural similarity is fortuitous. The molecular biology raises many fascinating problems relating to their structure and function. (shrink)

Recent discussions of this issue have centered on the definition of the of philosophy in American public life and the ways of increasing philosophy's influence in the public arena.' This emphasis is prompted by the fact philosophers are worried about the future of the profession. After a tremendous expansion in the sixties, there has been a steady decline in the number college-teaching positions open to newly graduated philosophers. The market is bloated and Ph.D.'s in philosophy have increasing difficulty securing permanent (...) jobs. The American Philosophical Association has tried to address this situation in various ways and discussions of the state and future of the profession, once rare, are becoming common. I am quite sure that part of the motivation for this panel involved these reasons. Today, however, I am not going to address the issue of jobs, or the ways which we can increase the influence of philosophy. Rather, I am going concentrate on only two questions: First, Does philosophy have a place contemporary American public life? Second, Should philosophy have a place American public life? Because my answer to the first question is going to be negative, I am also going to discuss some of the reasons why I believe philosophy does not play a role in American public life. (shrink)

Descartes is always concerned about knowledge. However, the Galileo affair in 1633, the reactions to his Discourse on method, and later his need to reply to objections to his Meditations provoked crises in Descartes’s intellectual development the import of which has not been sufficiently recognized. These events are the major reasons why Descartes’s philosophical position concerning how we know and what we may know is radically different at the end of his life from what it was when he began. We (...) call this later position Descartes’s epistemic stance and contrast it with his earlier methodological, metaphysical realism. Yet Descartes’s epistemic views cannot be separated from other aspects of his work, for example, his views concerning God, causality, metaphysics, and the nature of science. A further meta-implication is that serious errors await any scholar who cites early Cartesian texts in support of late Cartesian positions, or who uses later texts in conjunction with early ones to support a reading of Descartes’s philosophy.Keywords: René Descartes; Nature; Motion; Method; Knowledge. We argue that Isaac Newton really is best understood as being in the tradition of the Mechanical Philosophy and, further, that Newton saw himself as being in this tradition. But the tradition as Newton understands it is not that of Robert Boyle and many others, for whom the Mechanical Philosophy was defined by contact action and a corpuscularean theory of matter. Instead, as we argue in this paper, Newton interpreted and extended the Mechanical Philosophy's slogan “matter and motion” in reference (...) to the long and distinguished tradition of mixed mathematics and the study of simple machines. Dynamical collapse models embody the idea of a physical collapse of the wave function in a mathematically well-defined way. They involve modifications to the standard rules of quantum theory in order to describe collapse as a physical process. This appears to introduce a time reversal asymmetry into the dynamics since the state at any given time depends on collapses in the past but not in the future. Here we challenge this conclusion by demonstrating that, subject to specified model constraints, collapse (...) models can be given a structurally time symmetric formulation in which the collapse events are the primitive objects of the theory. Three different examples of time asymmetries associated with collapse models are then examined and in each case it is shown that the same dynamical rule determining the collapse events works in both the forward and backward in time directions. Any physically observed time asymmetries that arise in such models are due to the asymmetric imposition of initial or final time boundary conditions, rather than from an inherent asymmetry in the dynamical law. This is the standard explanation of time asymmetric behaviour resulting from time symmetric laws. (shrink)

What are physical quantities, and in particular, what makes them quantitative? This book presents an original answer to this question through the novel position of substantival structuralism, arguing that quantitativeness is an irreducible feature of attributes, and quantitative attributes are best understood as substantival structured spaces.
